  • This form should be filled and submitted through your higher authority to the appointing authority in 4/5 Sets along with relevant documents and Medical Certificate.
  • The appointing authority sends this form to the investigating authorities (police department)
  • After completion of the verification, Id Number is allotted stating that your antecedent verification is successfully.
  • Basing on this ID Number, the appointing authority issues a proceedings that your antecedent and characters verification has been successfully completed. This will be noted in the Service Register of the Individual.
